The wavy letters/words can be hard to read for any of us, add poor vision and the site is inaccessible. One way is to add a random control so that if they can't read it they can try others. Beyond that is a simple math problem or other question " 2+3=?" or "What color is the sky?" The issue here is that what you think is easy may not be for a nervous or hurried person with mental issues. So another way is a recorded group of letters or words, they hear it and write it in. That leaves the issue of people with poor hearing or trouble remembering random information long enough to repeat it in the box unless they can replay it and the box is open in front of them. Of course accents can be an issue for those not using the accent or it is not the mother language. But now we are back to their being other alternatives like those above, so it would be rare to find someone with all those issues. Just be sure there is alternatives.
