You may need to look at the formmail.cgi documentation to figure out the correct path to use. I'm not sure if the "/public_html" is necessary... again, check the documentation.

 

I can't seem to get a form to go on a page.

You have a submit button that submits the form, correct?

In that case, the 404 error indicates the path is incorrect somehow. I did notice that the URL has an extra slash in it next to "public_html" but again, you'll need to go back to the documentation to resolve this.

If you are getting a 404 error, the path cannot be right. 404 means the file is not at the location where you are looking for it.

 

As I've said multiple times in this topic, you need to look at any documentation your mail script has and/or talk to your web host.
