order form / multiple dropdowns

I have a question regarding the set up of an order form. I am actually using the forms module in CMSmadesimple but for the life of me can't figure out how to do this.

What I want is for the person to be able to order multiple clothing articles in different sizes if required. can someone help me with this. Here is a link to what I have and it is just not right yet.

looks like you figured out how to use the select field type in Formbuilder. I'm not sure if it is the right match for a shopping system though. You might want to check out some of the ecommerce modules for CMSMS.
